AB0093 CLINICAL AND SEROLOGICAL CHARACTERISTICS OF SYSTEMIC SCLEROSIS: EXPERIENCE FROM A TERTIARY CARE CENTER IN PAKISTAN

Background: Systemic sclerosis (SS) is less studied Connective tissue disease in our population. It characterized by different manifestations which if left undiagnosed and untreated lead to serious complications. The hallmark of this fibrosis various organs including skin also involving pulmonary, gastrointestinal cardiovascular system. We aim evaluate the clinical serological characteristics SS In addition, we evaluated prevalence Fibromyalgia Depression patients with SS. Objectives: aimed determine Clinical Serological Characteristics Sclerosis visiting department Rheumatology, Fatima Memorial hospital, Lahore. Methods: a cross-sectional study 38 outdoor indoor Rheumatology Hospital from September 2019 January 2020. All systemic age 16 years or above were included study. Skin tightness proximal elbow present 9 (23.7%). Microstomia (decreased mouth opening) 32(84.2%) Twenty-eight (73.7%) arthritis.Respiratory symptoms shortness breath 36(94.7%), palpitation 24 (63.2%) chest pain 7(18.4%). Gastrointestinal (GI) regurgitation 31 (81.5%) dysphagia 14 (36.8%). Interstitial lung 30 (78.9%) patients, higher diffuse scleroderma (100%) than limited (70%) (p=0.01). Pulmonary hypertension 18 significantly (57.1%), that secondary interstitial it (11.8%) (p<0.01). Thirty have restricted on pulmonary function tests. Obstetric history showed primary infertility at least 6 (15.8%) as compared (23.5% vs 9.5%, p=0.05). diagnosed per ACR criteria 7 (18.4%) depression assessed Anxiety (HADS) score 10 (26.3%) Anti nucleic acid antibody (ANA) positive Scl-70 antibodies significant association (85% 35.3% p<0.01), while anti centromere 20 (52.6%) patients; (94.2% 19.0%, p<0.01). Conclusion: Scleroderma very important, autoimmune multisystem disease. has preponderence. most initial feature followed other variable course severity. important complication poor prognosis.
